The history begins in 2011 where a perianal abscess first appeared, which was surgically treated with drainage and hospitalization. After recurrence of the abscess repeated interventions of abscess-drainage were needed by a fellow surgeon in a private practice.
Since then the patient remained asymptomatic, until 2018 a perineal abscess appeared and was automatically drained.
In August 2019, he again had a recurrent abscess at 7 o’clock , which had to be surgically drained but recurred a few weeks later.
After then the patient reports an outflow of purulent fluid in the perianal region and a change in toilet habits in recent days (constipation). No gas or fecal incontinence was detected.
Clinical Examination in lithotomy position:
Proctitis with peripheral eczema. External opening at 7th-8th hour in lithotomy position, with serous outflow.
Digital Examination:
Muscle tone of the External Sphincter felt adequate. Painful palpable mass in the seventh to eighth hour in lithotomy position.
